In the older patients, engaging in anal sex constitutes an increased risk factor for the development of this problem. Epididymitis is often one sided and it is usually the lead point of the larger disease of epididymo-orchitis. If it is epididymo-orchitis, however, antibiotic treatment lasting for several days usually deals with the infection. If that is all the problem you have, simple pain relievers should be helpful. But as you say the pain is severe on one side, perhaps there is an inner problem on that side that requires treatment.
